Access to Medical Records
In accordance with the Data Protection Act 2018 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the Reception team in writing who will then pass it on to our administration team. Please note that we will not be able to process any request unless it is made in writing.
In your letter you should state what dates you need your medical records between and a reason for your request if applicable.
We will comply with your request within 30 days. No information will be released without the patient consent unless we are legally obliged to do so.
What you need to know about your GP online records
Wouldn’t it be great if you could look at your GP records online?
Well, you can! You can also book and cancel appointments at your GP surgery and order repeat prescriptions online. You can already see some of the information in your GP online records, including your medications and allergies. During the next year, you will be able to see even more. This will include illnesses, immunisations and test results. Each GP surgery will make this information available at different times, as their computer systems become ready.
Just like online banking, you can look at your GP records on a computer, a tablet or a smartphone, using a website or an app. If you would like to start using online services, see the Getting Started with GP Online Services guide for more information. What’s in it for you?
• You can look at your records whenever you choose to, without needing to print them. Online records are up to date and more secure than a printed paper record which could get lost or seen by others.
• People who have long term conditions, for example diabetes, hypertension or coronary heart disease, have found that looking at their test results online helps them make positive changes to improve their health. They can see if their condition is improving or getting worse by looking at past test results. Adam, a patient at University Health Centre said ‘Record access is
useful for those, like me, who need to have more regular contact with their GP’.
• You can look at your medical records before your appointment to see if there is anything you need to discuss with your doctor or nurse. This could be your test results, illnesses you have had in the past or any new information added to your records. This would help you discuss any concerns you may have and help you benefit more from your appointment.
• Sometimes when you see your doctor, you are given a lot of information and might not be able to remember it later. You may also want further information once you have had time to think about what was said. You can look at your online records after your appointment to make sure you understood what your doctor or nurse said.
• One of the most useful things patients have found is that you can make sure your medical information is accurate. For example, you will be able to let your doctor know if you have an allergy to a medicine and it is not recorded.
• Before you go on holiday, you can check if your vaccinations are up to date without having to go to your surgery.
Understanding your records
Your records are written to help medical people look after you and so in some cases, you may not understand everything you see. If you find anything difficult to understand, as well as talking to your doctor or nurse, you can go to the NHS Choices website by using this link www.nhs.uk. NHS Choices is the NHS website for patients so you can look for information on illnesses, improving health and to find NHS services in your local area. Other websites frequently used to search for information on illnesses and test results are Patient – www.patient.info and Lab Test Online UK – www.labtestsonline.org.uk. Although these are not owned or checked by the NHS, other patients have found them useful.
A few things to think about
There are a few things you need to think about before registering for online records. On very rare occasions:
• Your GP may not think it in your best interest for you to look at your GP records online. If this happens, your GP will discuss their reasons with you. It is up to your GP to decide if you should be allowed access to your online records.
• You may see your test results before your doctor has spoken to you about them. This may be when you cannot contact your surgery, or when your surgery is closed. This means you will need to wait until an appointment is available to talk to your doctor.
• Information in your medical records might need correcting. If you find something you think is not correct, you should contact your surgery. The staff will be able to answer your questions and set things right when needed. Please bear in mind that you cannot change the record yourself.
• There may be information in your medical records that you did not know was there or that you had forgotten about, such as an illness or an upsetting incident. If you see anything you did not know about that worries you, please speak to your surgery and they will discuss this with you.
• If you see someone else’s information in your record, please log out immediately and let your surgery know as soon as possible.
• If you have questions about any of the above points, please talk to your surgery and they will be able to advise you further.

How to complain:
We hope that most problems can be resolved easily and quickly, often at the time they arise and with the person concerned. If you still wish to make a complaint, please inform a member of the practice as soon as possible. The time constraint of raising a complaint is 12 months from the occurrence, or 12 months from the time you become aware of the matter, about which you wish to complain.

What happens next?
A Manager will acknowledge your complaint within 3 working day’s. We will endeavour to conduct a full and thorough investigation in the shortest time possible, we aim to respond within 35 day’s. If the investigation is likely to exceed this time scale we will contact you and update you with all the progress to date.

Data Sharing
From 27th May 2019, the practice has changed the way we share information with certain NHS Organisations.
Currently, the Practice only shares information with other health providers, such as District Nurses with your consent; this can be prob-lematic as the District Nurses provide a 24-hour service every day of the year, and the Practice is not always open to provide record ac-cess should they require this.
The Practices in the Coastal Locality (Teignmouth and Dawlish) are planning to change the way we share data so that records will auto-matically be “shared out” with certain Organisations who will be on an approved list. These will be the other 3 practices within the Locality Glendevon Medical Centre, The Barton Surgery (Dawlish) and Teignmouth Medical Group, as well as Cricketfield Surgery in Newton Abbot who provide the Improved Access service to our patients, and the Torbay and South Devon Community Organisations who provide District Nurse, Community Phlebotomy, Intermediate Care, Community Physiotherapy and Occupational Therapy to patients who are registered with Practices in Teignmouth and Dawlish.
These other organisations will only be able to view your medical information for the purpose of direct care, for example, if they were visiting you at home following a hospital admission. Medical record access is audited, and staff working for the organisations listed above are bound by the strict confidentiality and data protection laws that apply to all NHS organisations.
The lawful basis for this method of sharing is defined under the General Data Protection Regulations are Article 6.1 (e) and 9.2 (h):
Article 6(1)(e) ‘…necessary for the performance of a task carried out in the public interest or in the exercise of official authority…’.
Article 9(2)(h) ‘necessary for the purposes of preventative or occupational medicine for the assessment of the working capacity of the em-ployee, medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems and services…”
This new way of working does not mean that your medical record will be open for anyone working in the above organisations to view. In order to view your record, you would have to be registered to receive care or a service from one of the organisations listed above; and the staff accessing your medical records would only be doing so for the purpose of providing direct care. If you are not registered with the organisation to receive care/services then your record will not be shared.
Record sharing in this way is an extremely useful tool, as it enables NHS organisations to view your GP record, and add to it. This means that your GP will be able to view the notes from the Community Nursing Team, Intermediate Care / Therapies Team, as well as any notes from the Improved Access service, all in one place. It also means that your GP record will be available for to these teams should you be referred / require care from them in the future. In addition to this, communication between these NHS organisations is streamlined as there is a common record available to the Health Professionals involved in your care. This sharing mechanism also means that you should only have to tell your story once, instead of having to repeat your medical history several times over when a new team is involved in your care.
Please note that if you have previously dissented (opted-out) to sharing your records, this decision will be upheld, and your record will only be accessed by your GP Practice. Should you wish to opt-out of the Enhanced Data Sharing Model (EDSM), please speak to a Re-ceptionist who will be able to update your personal preferences. Please note that by opting out of the EDSM, other health professionals may not be able to see important medical information, which may impact on the care you receive.
All other organisations using the computer system SystmOne, who are not listed above, will need to obtain a verification code from the patient in order to access their medical record subject to your consent. This method is very similar to that used as part of the Internet Banking process. The code will be sent to the patient via text, email, or obtained by logging in to their personal SystmOnline account.
GP Net Earnings
GP Earnings
All GP Practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.
The average pay for GPs working at Channel View Medical Group in the last financial year was £93,348 before tax and National Insurance. This is for 3 full time GPs, 9 part time GPs and 2 locums who worked in the practice for more than 6 months.
Improved Access

Monday— Thursday Improved Access Service
The practice offers additional evening appointments for our patients. These are different to the appointments some surgeries in the South Devon and Torbay CCG area currently offer in the evenings. This is a requirement (known as Improved Access) where practices ensure that across the week Monday-Thursday that a practice in their area is open between 6pm-8pm. To access these appointments please continue to contact your registered practice.

Weekend Improved Access Service
We also offer weekend appointments situated in a central hub location. The hub location for our patients is Albany Surgery Newton Abbot TQ12 2AS. Albany Surgery has been selected as the “weekend hub” as it is considered the most suitably based surgery geographically to serve 290,000 patients in the South Devon and Torbay area.
The appointments offered are by pre-booked appointments by contacting your local surgery and booking ahead. Patients will be able to be seen by a healthcare professional during these extended hours, we will be offering appointment’s with a GP, Practice Nurse and Health Care Assistant .
This service is very different to the current out of hour’s service where patients contact 111 in an urgent/acute situation that cannot wait until their practice is open again on the Monday. For any acute illness please still continue to contact 111, this new service is just for pre-booked routine work and is not for acute/urgent/walk in situations.
Please note that as this contract is being delivered at scale across the locality, you will not necessarily be seen by a Clinician known to you from your practice.

Please note as this contract is being delivered at scale across the locality, you will not necessarily be seen by a GP, Practice Nurse or HCA known to you from your practice.
To ensure that the Clinician you see has full access to your medical records, when you book your appointment the Receptionist will ask you to consent to sharing your medical records. Once consent is gained the clinician seeing you on the day is the only person who will have access to your medical record, this consent is valid for 28 days from the appointment date. It is essential that consent is gained for you to be able to have the appointment. If you feel you do not wish to provide consent then it will not be suitable for you to book the appointment.

Free Surgery Parking is available for up to a maximum of 1 hour in front of the Sainsbury’s Local Store, please do not rely on this as spaces are limited.
Medical Education
We are a training practice and you may be offered an appointment with a GP registrar. These doctors are fully qualified and have had experience in several hospital specialties. They are completing their specialist training as a GP.
We also teach medical students and sometimes you may be asked to see the doctor or nurse with a student present. You will always be asked beforehand if this is acceptable and you may ask for the student to withdraw if you prefer to have your consultation in private.

GPConnect and Enhanced Data Sharing Module We share your record using GP Connect and the Enhanced Data Sharing Module to make sure that, whether you are visiting the practice, attending hospital, or being seen in the community or at home by a care professional, everyone knows the care you need and how you want to be treated. Your electronic health record is available to the practices in Coastal Primary Care Network and other local providers via GPConnect/SystmOne who are involved in your care. This includes the sharing of personal contact details, diagnosis, medications, allergies, and test results. Summary Care Record (SCR) NHS England have implemented the SCR which contains information about you; including your name, address, data of birth, NHS number, medication you are taking and any bad reactions to medication that you have had in the past. This information is automatically extracted from your records and uploaded onto a central system. Many patients who are seen outside of their GP Practice are understandably not able to provide a full account of their care or may not be in a position to do so. The SCR means patients do not have to repeat their medical history at every care setting and the healthcare professional they are seeing is able to access their SCR. The SCR can only be viewed within the NHS on NHS smartcard-controlled screens or by organisations, such as pharmacies, contracted to the NHS. As well as this basic record, additional information can be added to include further information. However, any additional data will only be uploaded of you specifically request it and with your consent. You can find out more about the SCR here: https://digital.nhs.uk/services/summary-care-records-scr National Screening Programmes The NHS provides national screening programmes so that certain diseases can be detected at early stages. These screening programmes include bowel cancer, breast cancer, cervical cancer, aortic aneurysms and a diabetic eye screening service. More information on the national screening programmes can be found at: https://www.gov.uk/topic/population-screening-programmes One Devon Dataset As well as using your data to support the delivery of care to you, your data may be used to help improve the way health and social care is delivered to patients and service users throughout Devon using Population Health Management methods. We will use a pseudonymised extract (i.e., not identifiable information) which will be sent securely to NHS Devon ICB (Integrated Care Board) and in partnership with the Local Authorities. Data will be used to support the Devon Integrated Care System to improve short-term and medium-term health outcomes for local populations. If you would benefit from some additional care or support, your information will be shared back to the practice, or another local provider involved in your care, so that they can offer you direct care. If you have previously asked the practice to apply a Type 1 opt-out to your medical records, this will be applied by NHS Devon ICB. Devon and Cornwall Care Record Health and social care services in Devon and Cornwall have developed a system to share patient data efficiently and quickly and, ultimately, improve the care you receive. This shared system is called the Devon and Cornwall Care Record. It’s important that anyone treating you has access to your shared record so they have all the information they need to care for you. This applies to your routine appointments and also in urgent situations such as going to A&E, calling 111 or going to an out-of-hours appointment. It’s also quicker for staff to access a shared record than to try to contact other staff by phone or email. Only authorised staff can access the Devon and Cornwall Care Record and the information they see is carefully checked so that it relates to their job. Also, systems do not share all your data – just data that services have agreed is necessary to include. Employees of the practices will access your information in order to determine whether you are suitable to be invited to participate in a study. We only hold your data for as long as necessary and are required to hold your data in line with the NHS Records Management Code of Practice Retention Schedule. What rights do you have? Please note that you are entitled to a copy of your data that we hold free of charge; however, we are entitled to charge in certain circumstances where the law permits us to do so. Right to restrict or object the use of your information: There are certain circumstances in which you can object from your data being shared. Information regarding your rights to opt-out is detailed below: Consent: If the practice is relying on the consent as the basis for processing your data, you have the right to withdraw your consent at any time. Once you have withdrawn your consent, we will stop processing your data for this purpose. However, this will only apply in circumstances on which we rely on your consent to use your personal data. Please be aware that if you do withdraw your consent, we may not be able to provide certain services to you. If this is the case, we will let you know. Summary Care Record: The SCR improves care; however, if you do not want one, you have the right to object to sharing your data or to restrict access to specific elements of your records. This will mean that the information recorded by the practice will not be visible at any other care setting. You can also reinstate your consent at any time by giving your permission to override your previous dissent. National Screening Programmes: If you do not wish to receive an invitation to the screening programmes, you can opt out at https://www.gov.uk/government/publications/opting-out-of-the-nhs-population-screening-programmes or speak to the practice. Type 1 Opt-out: You have the right to object to your confidential patient data being shared for purposes beyond your direct care by asking the practice to apply a Type 1 opt-out to your medical records. A type 1 opt-out prevents personal data about you, being extracted from your GP record, and uploaded to any other organisations without your explicit consent. National Data Opt-out: You have the right to object to your data being shared under the national data opt-out model. The national data opt-out model provides an easy way for you to opt-out of sharing data that identifies you being used or shared for medical research purposes and quality checking or audit purposes. This applies to matters of fact, not opinion. If the information is of clinical nature, this will need to be reviewed and investigated by the practice. If your personal information changes, such as your contact address or number, you should notify the practice immediately so that we can update the information on our system. We will also ask you from time to time to confirm the information we hold for you, is correct. Right to erasure: The practice is not aware of any circumstances in which you will have the right to delete correct data from your medical record, which the practice is legally bound to retain. Although you are free to obtain your own legal advice if you believe there is no lawful purpose for which we hold the data and contact the practice if you hold a different view. Data outside EEA We do not send your personal data outside of the EEA. However, if this is required, the practice would only do so, with your explicit consent. I acknowledge that appointment reminders by text are an additional service and that they may not be sent on all occasions but that the responsibility for attending appointments or cancelling them still rests with me. I can cancel the text message facility at any time.
Text messages are generated using a secure facility but I understand that they are transmitted over a public network onto a personal telephone and as such may not be secure, however the practice will not transmit any information which would enable an individual patient to be identified.

The Coastal Primary Care Network
Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.
To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).
PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.
