How can the Wellbeing Link Worker support you?
You might wish to refer yourself to the Social Prescribing Scheme for the following reasons:
- you are unemployed and want to seek employment
- you are experiencing financial difficulties and require support
- you are experiencing housing difficulties and require support
- you are experiencing low-moderate mood and want this to improve
- you are isolated and want this to change
- you are struggling with your mobility and would like to explore the available options of support
- you’ve experienced a bereavement and are struggling to move forward
…or you might have another health and wellbeing need that the Wellbeing Link Workers could potentially support you to improve.

Once completed a Wellbeing Link Worker will attempt to make contact with you within 2 working weeks. Within this call, the Wellbeing Link Worker will explain the support they can offer you in more detail and will arrange a date and time for an initial face to face or telephone appointment with you.
Self-Referrals for the Carer and Dementia Support Social Prescribing Scheme
How can the Carer and Dementia Support Wellbeing Link Worker support you?
Do you have a friend or family member that relies on you for their care? This could be taking them to appointments, helping them with cleaning, laundry, dealing with finances or correspondence, picking up medication, checking in on them AND OR helping them with personal care such as helping or supervising them to wash, dress or brush their hair?
If so, you might wish to refer yourself to the Carer and Dementia Support Social Prescribing Scheme for the following reasons:
- You are finding it difficult to maintain your carer responsibilities and need support
- You are new to your caring role and would like to understand what support is available to you
- You’ve recently been diagnosed with Dementia and would like to understand more about the practical support that’s available for you and your family
- You have a diagnosis of Dementia and your symptoms are getting worse and you are finding to difficult to cope with the practical elements of your life
…or you might have another health and wellbeing need related to your caring responsibilities or Dementia Diagnosis that the Wellbeing Link Workers could potentially support you to improve.
